Notice to those using Eurofins for compliance testing

This message is going out to any water systems that are using or plan to use a Eurofins lab for water testing compliance. There have been many cases where the lab results are not getting submitted to the Department of Health which can trigger a violation notice. All water systems doing compliance testing at a Eurofins lab must submit a Washington Compliance Info Sheet.

In addition a second form is required for all water systems but which form used depends on whether your water system has a Eurofins account or not.

All water systems doing compliance testing at Eurofins lab WITH a Eurofins account must submit a Eurofins Chain of Custody (COC) form. Or those water systems doing compliance testing at Eurofins lab WITHOUT a Eurofins account must submit a DWWW PPS form.
